We are offering qualified and experienced candidates a sign-on bonus not to exceed 10% of the salary for this position.Position: Director of Care Coordination at St. Mary Medical CenterThe Director of Care Coordination will be responsible for the overall direction and management of care coordination and social services. Key responsibilities include:

Planning, organizing, and directing all activities related to care coordination and social services.
Managing staffing and performance improvement initiatives, particularly in the delivery of clinical services such as Length of Stay (LOS) reduction.
Overseeing reporting needs within the Medical Center, Dignity Health, and compliance with government and regulatory agencies.
