Job Description:

A company is looking for a Health Outreach Associate with sales experience.
Key Responsibilities

Conduct initial screening consultations and assessments with patients and caregivers
Coordinate sessions with medical providers and home care aides
Increase patient engagement and retention through compassionate outreach

Required Qualifications

Experience in a healthcare environment, preferably with dementia or Alzheimer's patients
Sales mindset with a goal-oriented approach
Ability to work East Coast hours
Tech-savvy with troubleshooting skills for remote work
Multilingual abilities are a plus
