Job Description HonorVet Technologies is seeking a travel nurse RN Case Management for a travel nursing job in Mission Viejo, California. Job Description & Requirements Specialty: Case Management Discipline: RN Start Date: 12/01/2025 Duration: 13 weeks 40 hours per week Shift: 8 hours, days Employment Type: Travel Unit Description – Case Management Department Unit Type: Inpatient Case Management Workload & Census: Caseload: up to 25 patients per day Shift & Scheduling: 8-hour shifts: 08:00 AM – 04:30 PM Schedule varies based on unit needs (4-week schedule) Float required Call required Weekend coverage: every other weekend (varies with department needs) Key Notes: Epic is a HARDSTOP requirement Case Managers do NOT perform Utilization Review Responsibilities & Daily Functions Care Coordination & Discharge Planning Full patient assessments and treatment plan development Coordinate discharge planning including: SNF DME Home health Ambulance transport Independently manage safe discharge workflows Organize and participate in family meetings Collaborate with payor systems including HMOs, IPAs, Medicare/Medi-Cal Support transitions of care and community resource planning Special Populations Experience: Adoption/foster care Substance use disorders Homelessness Patients with psychiatric history or crisis involvement Advance directives, POLST Protective services reporting and coordination Documentation & Technology: Epic required Computerized charting proficiency required Additional checklist required Required Background & Experience Minimum 2 years acute medical Case Management experience (HARDSTOP) Must include: Care coordination Discharge planning Travel experience required (first-time travelers not accepted) Strong competency with Microsoft applications Experience with DME ordering, SNF placement, ambulance arrangements No UR responsibilities in this role Support Services Available Social Services Pharmacy Physical Therapy Respiratory Therapy Radiology Rapid Response IV Teams Lift Teams Interpreter Services Transportation Unit Secretary support Certifications BLS – Required Orientation & Other Details Orientation: 40 hours Dress code: Turquoise scrubs Pre-approved time off: 1 request per contract About HonorVet Technologies HonorVet Technologies: Pioneering Purpose, Empowering Heroes Founded in 2015, HonorVet Technologies stands as a Service-Disabled Veteran-Owned Small Business (SDVOSB) , proudly certified by NaVOBA and the U.S. Department of Veterans Affairs .
