facilities are Intermediate Care Facilities for individuals with
disabilities (ICF/DD-H and ICF/DD-N) Caregiver (DSP) to people
supported ratio is usually 1:3, sometimes as high as 1:1 support
Nursing caseload typically ranges from 6-15 residents Our home
sizes are small, including 6-bed and 15-bed facilities High-acuity
care is limited or prohibited by facility regulations Our Immediate
Care Facilities provide wound care per licensing requirements,
ranging from simple wounds to a maximum of stage I pressure ulcers
Hospice care is very limited and is provided collaboratively with
professional hospice care providers who come to our homes Momentum

staff. As a Licensed Vocational Nurse (LVN ) at Momentum, you will
be an integral part of our team, providing high-quality nursing
care and support to individuals with disabilities in our
Intermediate Care Facilities (ICF). This role offers you the

build meaningful relationships with both the residents we support
and your fellow dedicated team members. Licensed Vocational Nurse
(LVN) Essential Responsibilities: Provide skilled nursing care to
individuals with disabilities in an Intermediate Care Facility
setting Implement and oversee the health care plans for residents
Administer medications and perform medical procedures as needed
Collaborate with a multi-disciplinary team to ensure the well-being
of residents Maintain accurate medical records and documentation
Offer support and guidance to Direct Support Professionals (DSP)
Participate in complex case management and engage in continuous
learning opportunities Licensed Vocational Nurse (LVN)
Qualifications: Valid License Vocational Nurse (LVN) license in the
state of California CPR certification Successfully pass all
post-offer, pre-employment screening requirements, including
criminal background check, drug screening, and provide proof of
eligibility to work in the United States Ability to run, push,
pull, bend, squat, kneel, lift up to 50 lbs., and transfer
individuals Strong communication and interpersonal skills A genuine
passion for providing quality care and support to individuals with
disabilities Ability to work effectively in a team-oriented
environment Willingness to adapt to the needs of the residents and
