Job Description:
Title: Care Manager – Medical Social Worker Schedule: - Typical
schedule: Sunday–Thursday - Weekend requirement: Every other
weekend - Holiday requirement: Must commit to working 3 of the
following: Thanksgiving, Christmas Eve, Christmas Day, New Year’s
Eve, New Year’s Day Assignment Details: - Caseload: 20–23 patients
Requirements: - Minimum of 1 year of acute care experience required
- Experience in case management/discharge planning - Valid LCSW or
MSW credential - BLS certification - Proficiency in EPIC charting
system Roles & Responsibilities: - Conduct initial assessments and
care coordination for adult and geriatric patients. - Develop and
implement discharge plans, ensuring smooth transitions to
post-acute services. - Liaise with medical teams and utilization
management case managers to align patient care plans. - Provide
crisis intervention and high-risk patient management. - Coordinate
referrals to home care, durable medical equipment, and community
resources. - Maintain documentation consistent with care plans and
coverage criteria. Compliance / Notes: - Orientation: 40 hours -
Must be willing to float to partner facilities within the state -
Pre-approved time off: 1 day - RTO policy allows up to 7 days RTO -
