Job Description:
A company is looking for a Pre-Visit Care Connector to work
remotely in multiple locations.
Key Responsibilities
Conduct telephonic outreach to members and assist with Pre-Visit
Assessments
Document demographic and medical history information accurately
Provide excellent customer service and maintain compliance with
HIPAA regulations
Required Qualifications
High School Diploma/GED or equivalent experience
At least 2 years of call center or telephonic customer service
experience, preferably in healthcare
2+ years of healthcare experience or social work/community outreach
experience
1+ years of experience in collecting and documenting medical
history information
Proficient in Microsoft Word, Excel, and Outlook
